Clinic-to-clinic transfers

End-to-end management of a transfer between two clinics, including the administrative sequence that has to be completed before a date can be agreed.

Most delays in a transfer are administrative, not logistical. Consent forms are outstanding, the receiving clinic has not yet confirmed capacity, or a release cannot be authorised by the person who asked for it.

This service exists to take that sequence off the clinics and the patient. We identify what is missing, tell the responsible party exactly what is needed, and hold the schedule open until the position is clear.

How coordination works

We produce a written checklist showing what each party has to provide and by when.

We chase outstanding items directly rather than asking the patient to do so.

We confirm the date only once the checklist is complete.

Before collection

  1. Map the sequence and identify the responsible party for each step
  2. Issue the checklist to the patient and both clinics
  3. Track outstanding items to completion
  4. Confirm the collection and delivery windows once the position is clear

During transport

  • A single case reference is used by everyone involved
  • Both clinics are notified at collection and before arrival
  • Questions from either clinic are answered by the transport team, not passed to the patient

At handover

  • The receiving clinic confirms receipt against the documentation
  • The custody record and the completed checklist are issued together
  • The case reference is closed and confirmed to all parties

  • The names of both clinics and the patient reference each holds
  • Who has authority to instruct the transfer
  • Any date the transfer has to fit around
  • Whether the receiving clinic has already confirmed it will accept the material

Limitations

Where this service stops

Stated up front, because finding out later is worse for everybody.

  • We cannot complete consent or release forms on behalf of a patient or a clinic.
  • A clinic may decline to accept material for its own reasons, which we cannot override.
  • Timescales depend on how quickly outstanding items are provided.
